DHS Form 7001, Request for Case Assistance, is used to request case assistance from the Office of the Citizenship and Immigration Services Ombudsman (CIS Ombudsman). Before you submit this form:
- Please contact U.S. Citizenship and Immigration Services (USCIS) first before submitting a request with our office, and
- Review the types of cases we can and cannot help with.

We strongly prefer that you submit the DHS Form 7001 online with supporting documentation, the dates you contacted USCIS, and all USCIS service request (SRMT) numbers. Submitting your request online allows us to process it more efficiently. If you cannot submit the DHS Form 7001 online, the second best option is to email a completed DHS Form 7001 to us at cisombudsman@hq.dhs.gov. If you choose to mail the form to us, it may be significantly delayed due to security measures with the government mail system (even for express mail).
